Our new patient forms can be printed from this site and completed by you prior to your visit. If you choose to print them now, bring the completed forms with you at the time of your appointment. We will be happy to answer any questions you might have at that time. Be sure to bring your insurance card with you so that we may get your insurance information correct in our system. If your insurance requires a referral from your primary care physician, please obtain that referral PRIOR to your appointment. If you fail to bring a referral it may be necessary for us to reschedule your appointment. If you are not covered by health insurance we require a $100.00 payment at the time of your visit and we will be happy to work out a payment plan with you at that time.
